| 目的 研究镇江地区食源性沙门氏菌的时空分布及耐药性。方法 2021—2024年根据《江苏省食源性疾病监测工作手册》对镇江市4个市区哨点医院的食源性腹泻样本(粪便)开展沙门氏菌的分离培养及血清学分型, 并采用微量肉汤稀释法进行药敏试验。结果 2547份食源性腹泻样本中检出沙门氏菌 117株, 沙门氏菌的检出率为4.59%。其中, 夏秋季样本量较多, 阳性检出率较高(5.31%和4.75%), 不同季节沙门氏菌的检出率差异有统计学意义(χ2=6.61, P<0.05)。润州区和丹阳市(8.23%、7.77%)的沙门氏菌检出率显著高于其他地区(χ2=66.622, P<0.05), 镇江地区优势血清型为鼠伤寒沙门氏菌(45.30%)。本地区沙门氏菌的耐药率较高(82.05%), 对氨苄西林耐药率最高(82.05%), 其次是四环素(70.09%)和氨苄西林/舒巴坦(64.10%)、氯霉素(52.14%)等, 多重耐药率占75.21%, 多重耐药谱呈现52种, 多重耐药谱以氨苄西林-氨苄西林/舒巴坦-四环素-氯霉素-复方新诺明(12.50%)和氨苄西林-氨苄西林/舒巴坦-萘啶酸-多粘菌素E (7.95%)为主。结论 镇江地区食源性沙门氏菌夏秋季高发, 润州区和丹阳市检出率较高, 血清型多样化, 菌株呈现高耐药率及复杂的多重耐药谱。该结果为当地食源性疾病的精准预警、临床经验性用药指导以及抗生素耐药性监测政策的制定提供了关键科学依据。 |

| Objective To study the spatiotemporal distribution and drug resistance of foodborne Salmonella in Zhenjiang Region. Methods From 2021 to 2024, according to the Jiangsu Province foodborne disease surveillance work manual, stool samples from foodborne diarrhea cases were collected from sentinel hospitals in 4 urban districts of Zhenjiang City.?Salmonella?isolation and cultivation, serotyping and drug sensitivity tests (using the broth microdilution method) were performed. Results Out of 2547 samples of foodborne diarrhea, 117 strains of Salmonella were detected, with a detection rate of 4.59%. Among them, the sample size was larger in summer and autumn, with higher positive detection rates (5.31% and 4.75%). There was a statistically significant difference in the detection rate of Salmonella in different seasons (χ2=6.61, P<0.05). The detection rates of Salmonella in Runzhou District and Danyang City (8.23%, 7.77%) were significantly higher than those in other areas (χ2=66.622, P<0.05). The predominant serotypes in Zhenjiang Area were?Salmonella?typhimurium (45.30%). The drug resistance rate of Salmonella in this area was high (82.05%). The highest resistance was to ampicillin (82.05%), followed by tetracycline (70.09%) and ampicillin/sulbactam (64.10%), chloramphenicol (52.14%) and so on, and the situation of multidrug resistance was 75.21%, with a multidrug resistance spectrum of 52, mainly ampicillin-ampicillin/ sulbactam-tetracycline-chloramphenicol-sulfamethoxazole-trimethoprim (12.50%) and ampicillin-ampicillin/sulbactam- nalidixic acid-polymyxin E (7.95%). Conclusion Foodborne Salmonella in Zhenjiang region has a high incidence in summer and autumn, with relatively high detection rates in Runzhou District and Danyang City. The serotypes are diverse, and the strains exhibit high antibiotic resistance rates and complex multidrug resistance spectrum. These results provide a key scientific basis for precise early warning of local foodborne diseases, guidance for empirical clinical medication, and the formulation of antibiotic resistance monitoring policies. |
